Short PSA: Trans and non-binary people on medical forms

All medical forms have an input for gender, and most, if not all, present only the options Male and Female. While it may be difficult being misgendered, doctors need to know your sex as assigned at birth. The different sexes run different health risks, and this information is needed by medical professionals to know where and what to test for. For example, even if you presently identify as a female, if your birth sex was male, you still run the risk of prostate cancer. So, instead of crossing out the checkboxes and inserting your gender, please select your assigned sex at birth, for your own safety.

Note: This only applies to medical forms, this information is not vital on opinion surveys
